Как уходят в альтернативную медицину: кризис доверия в коммуникации врача и онкопациента

Аннотация

Статья фокусируется на специфическом ролевом противостоянии врача и пациента в окологии, которое варьируется от нормативно контролируемого до конфликтного и противоречивого. Осложненное взаимодействие, порождающее кризис коммуникации врача и пациента, а также дефицит доверия, выступают важными мотивами отказа пациента от конвенциональной онкотерапии. Этот кризис особо выражен в тех случаях, когда речь идет о терминальных заболеваниях, где потеря времени и неверное решение могут ухудшить перспективы излечения. Анализируются примеры таких кризисов коммуникации, вынуждающие к поискам альтернативного способа излечения. Реконструируя на основе нарративов о болезни взаимодействие между врачом и пациентом, мы описываем то, как пациенты представляют коммуникацию с онкологом и обосновывают отказ от конвенциональной терапии или, наоборот, возврат к ней, а также то, как онкологи воспринимают такие действия пациентов. Мы резюмируем нарративы по трем тематическим основаниям: риторические приемы легитимации патернализма у врачей, типологии стратегий борьбы пациентов с онкозаболеванием и нормативные представления с обеих сторон, которые формируют запрос на новую этику медицинской заботы.
